Medicinal products containing 5-fluorouracil (5-fluorouracil) (for intravenous use) – in patients with moderate or severe renal impairment, phenotyping of dihydropyrimidine dehydrogenase (DPD) deficiency by measuring blood uracil levels should be interpreted with caution

The European Medicines Agency (EMA) has published a Direct Healthcare Professional Communication (DHPC) containing important information for healthcare professionals who prescribe medicinal products containing 5-fluorouracil (5-fluorouracil) (for intravenous use).

Dear healthcare professional!

Registration certificate holders (RCHs) of medicinal products containing 5-fluorouracil (5-fluorouracil) (for intravenous use), in agreement with the European Medicines Agency (EMA) and [National Authorized Body], inform you of the following:

Brief overview

  • In patients with moderate or severe renal impairment, the blood uracil level used for phenotyping dihydropyrimidine dehydrogenase (DPD) should be interpreted with caution, since impaired renal function may lead to an increase in blood uracil levels.
  • Therefore, there is an increased risk of incorrect diagnosis of DPD deficiency, which may lead to underdosing of 5-fluorouracil and, consequently, to reduced treatment efficacy.

Prerequisites for assessing safety issues

Parenteral 5-fluorouracil (5-FU) is part of the standard therapy for various malignant neoplasms, including cancers of the colon, pancreas, stomach, breast, and head and neck. It is mostly used in combination with other antineoplastic medicinal products.

The rate-limiting enzyme of 5-FU catabolism is dihydropyrimidine dehydrogenase (DPD). As a result, patients with DPD deficiency have an increased risk of severe or life-threatening toxicity during treatment with 5-FU or one of its prodrugs, so phenotyping and/or genotyping is recommended before starting therapy.

To identify such patients, testing is recommended before the start of treatment, despite the uncertainty regarding the optimal testing method.

  • Patients with complete DPD deficiency are at high risk of life-threatening or fatal toxicity and must not receive therapy with 5-FU or other fluoropyrimidines (capecitabine, tegafur).
  • Patients with partial DPD deficiency have an increased risk of severe and potentially life-threatening toxicity. To limit the risk of severe toxicity, a reduction of the starting dose should be considered. Subsequent doses may be increased in the absence of serious toxicity, since the efficacy of the reduced dose has not been established.

<When choosing the appropriate approach to determining the activity of the DPD, national recommendations should be taken into account>.

[Additional information may be added in accordance with the national context, if necessary].
