The notification card on an adverse reaction to a medicinal product, vaccine or tuberculin, and/or lack of efficacy of a medicinal product, and/or an adverse event following immunization/tuberculin diagnostics is the form used by doctors, pharmacists, feldshers, midwives, pharmaceutical workers and nurses of all healthcare facilities, regardless of ownership type, to report any cases of adverse reactions to medicinal products, vaccines or tuberculin, lack of efficacy of medicinal products, and adverse events following immunization/tuberculin diagnostics.
The report card is submitted in paper and/or electronic form
User registration is not required to complete the report form electronically.
