To declare the wholesale selling price of a medicinal product, the holder of the registration certificate for the medicinal product (or the holder of a permit for parallel import of the medicinal product) or its authorized representative shall submit, in electronic form, an application for declaring the price of the medicinal product with an electronic signature affixed in accordance with the procedure established by law.
Together with the application, it is necessary to send a power of attorney to a legal/natural person representing the interests of a foreign or domestic applicant in Ukraine, with clearly defined powers, including the right to represent the interests of the company (company name) in the central executive body that implements state policy in the field of healthcare or a body authorized by it for the referencing of prices for medicinal products, declaration of prices for medicinal products in the National Price Catalogue, provision/receipt of information to/from the National Price Catalogue.
Documents shall be submitted as color scanned copies of all pages in chronological order with continuous numbering in PDF format. The scanned copies shall be clear and show all the details of the document (signatures, stamps, seals, etc.). The scanning resolution shall be at least 200 dpi
Documents and their copies issued outside the territory of Ukraine shall be notarized at the place of their issuance, legalized at a consular office of Ukraine in the respective foreign state (unless otherwise provided by international treaties to which Ukraine is a party) and translated into Ukrainian.
Legalization of documents issued in countries with which Ukraine has concluded relevant bilateral international agreements is not required, provided that the contracting parties observe the principle of parity, or if the country in which the above-mentioned documents were issued is a party to the Hague Convention (1961), which abolishes the requirement of legalization of foreign official documents (legalization by affixing an apostille).
A power of attorney issued by way of sub-delegation is subject to notarisation. Such a power of attorney is sent together with the main (general) power of attorney.
