Regarding amendments to the package insert of the vaccine for the prevention of varicella (live) [varicella vaccine (live)]

In accordance with paragraph 8 of Section IV of the Procedure for Expert Evaluation, the applicant must amend the instructions for medical use of the medicinal product upon receipt of new data, including bringing the instructions for medical use into line with the approved instructions for medical use of the original (reference) medicinal product, and ensure that the information on the medicinal product corresponds to current scientific knowledge.

Based on the analysis of periodic safety update reports (PSURs) for varicella vaccines (live) [varicella vaccine (live)], PSUSA/00010473/202503, the Pharmacovigilance Risk Assessment Committee (PRAC) reached the following conclusions.

Having considered the PRAC recommendations, the Co-ordination Group for Mutual Recognition and Decentralised Procedures – Human (CMDh) agrees with the overall conclusions of the PRAC and the grounds for the recommendations.

Based on the scientific conclusions regarding the varicella vaccine (live) [varicella vaccine (live)], the CMDh considers that the benefit/risk balance of the vaccine Varilrix [varicella vaccine (live)] remains unchanged, provided that the proposed changes are made to the package leaflet.

Changes to be made to the relevant sections of the patient information leaflet (new text is underlined and in bold, deleted text is struck through).

  • Section “Special warnings and precautions for use”

The warning regarding persons at increased risk of severe varicella should be amended as follows:

Persons at increased risk of a severe course of chickenpox

Clinical study data on the use of the vaccine Varilrix [varicella vaccine (live)] (+4°C) in persons at increased risk of severe varicella are limited.

Vaccination may be considered for patients with certain immunodeficiency conditions when the expected benefit outweighs the potential risks (for example, in persons with asymptomatic HIV infection, IgG subclass deficiency, congenital neutropenia, chronic granulomatous disease, and diseases associated with complement deficiency).

In immunocompromised patients without contraindications to vaccination (see “Contraindications”), the immune response to vaccination may be reduced compared with immunocompetent individuals, so some of these patients may contract varicella upon contact despite proper use of the vaccine. Such patients should be closely monitored for timely detection of signs of varicella.

If a decision is made to vaccinate persons at increased risk of severe varicella, the following recommendations should be followed:

  • maintenance chemotherapy in patients in the acute phase of leukemia should be suspended 1 week before and 1 week after immunization. In patients receiving radiotherapy, vaccination is generally not performed during the treatment period. Immunization is generally carried out during the period of complete hematological remission;
  • total lymphocyte count should be at least 1,2 × 10⁹/L or there should be no other signs of impaired cellular immunity;
  • vaccination of patients scheduled for organ transplantation (for example, kidney) should be carried out several weeks before the start of immunosuppressive therapy.

Changes to the safety information for the varicella vaccine (live) [varicella vaccine (live)] following the meeting of the Pharmacovigilance Risk Assessment Committee (PRAC) of 27 – 30 October 2025

The European Medicines Agency (EMA) has published the minutes of the meeting of the Pharmacovigilance Risk Assessment Committee (PRAC) held on 27 – 30 October 2025. Following the meeting, a decision was made on the need to introduce safety-related amendments to the registration materials/package insert of the vaccine for the prevention of chickenpox (live) [varicella vaccine (live)].

Vaccine for the prevention of varicella (live) [varicella vaccine (live)] – PSUSA/00010473/202503

Varicella vaccine (live) [varicella vaccine (live)] is a lyophilized product of live attenuated (weakened) varicella virus (Oka strain), intended for active immunization against varicella in healthy individuals from 9 or 12 months of age.

Based on the assessment of periodic safety update reports (PSURs), the PRAC reviewed the benefit/risk balance of vaccines for the prevention of varicella (varicella vaccine) and provided the relevant recommendations.

Summary of recommendations and conclusions

  • Based on the analysis of safety and efficacy data, the benefit/risk balance of vaccines for the prevention of varicella (varicella vaccine) (except Varilrix [varicella vaccine (live)]) for the approved indications remains unchanged. The existing provisions of the package leaflets should remain unchanged.
